To file a complaint against a psychologist please see the following documents for the procedure.

Title 17, Series 4

Below is the Ethics Inquiry Form needed to file a complaint against a psychologist.  You can contact the Board office at wvpsycholgybd@mail.state.wv.us for a complete ethical inquiry packet.  Please include your name, the address where you would like the packet mailed and include the name of the psychologist so that we can check to see if the licensee is in fact a licensed psychologist in WV.
